Medical Insurance and Health Information
Members of our Class of 2018 must submit a Medical History/Physical Examination Form, an Immunization Record, a Student Health Insurance Form, and an Authorization and Consent to Bill Insurance Form. These forms will be available in March of 2015.
W&L requires all students to carry personal health insurance and submit at the beginning of each year both an on-line waiver/enrollment form to First Risk Advisors and the above Health Insurance Form with a copy of all insurance cards to the Student Health Center. If you do not have health insurance coverage, you have the option of purchasing a student health insurance plan. You may access information about this plan at www.firststudent.com - select W&L from the drop down menu and click on "Review Brochures." In addition to submitting the three forms above, students will receive an email from First Risk Advisors later in the summer with information about the mandatory waiver/enrollment form, which you may complete online at www.firststudent.com.
Required health forms are due in the Student Health Center by July 31; questions should be directed to StudentHealth@wlu.edu.
Visit the University's Student Health Center's website for general information about campus health services, student health insurance options, etc.
An official final undergraduate transcript must be received by W&L Law directly from your undergraduate institution, no later than June 30. Your transcript must indicate that you have qualified for your undergraduate degree before registration in August; it is permissible for your degree to be conferred after registration if all other degree requirements are completed. Transcripts provided to LSDAS and transcripts issued to you, rather than directly to W&L Law, do not satisfy this requirement. If you have completed graduate work, we recommend you also provide your graduate school transcripts, but it is not required.
Please have your final transcript mailed to the following address:
